Refer a child for pediatric therapy
A Grade Therapy welcomes referrals from pediatricians, CDSA and early intervention coordinators, ABA organizations, schools and other professionals supporting children and families in Greater Charlotte. We help families explore pediatric occupational therapy, speech therapy and physical therapy, with current availability confirmed for each child's location and needs.
Which service does the child need?
- Pediatric occupational therapy supports participation in play, self-care and everyday routines.
- Pediatric speech therapy supports communication and related skills identified through evaluation.
- Pediatric physical therapy addresses movement and mobility needs. Contact us to confirm current PT availability before planning a start date.
How to make a referral
- Confirm coverage and capacity. Call 980-399-5454 to discuss the requested discipline, general service area and appropriate next step.
- Send referral documentation. Fax referrals and applicable orders to 980-422-0169. Include the requested service, reason for referral, family contact information and relevant supporting documents through this referral channel.
- Request family follow-up. Parents or guardians can use our request form to share their own contact information. Do not include child identifiers or medical information in the website form. Our team reviews the request and emails the appropriate secure intake documents, then coordinates any required orders, coverage verification or authorization before scheduling.
Therapy in the child's everyday environment
Explore our Charlotte and Concord service areas. Other existing AGT territories are reviewed case by case. Depending on the child's needs, therapist availability and site permission, care may take place at home, daycare, an ABA center or another appropriate community setting.
Availability varies by discipline, location and schedule. A referral or follow-up request does not guarantee immediate placement.
